Safety Alert: Protecting vulnerable road users around work sites

A cyclist was fatally injured in collision with a concrete truck on a road work site. Improvements to temporary traffic management have been reviewed and published as part of an enforceable undertaking following the incident.

Safety alert - safe trenching

A worker had to use his hands and a spade to dig his workmate free from a torrent of dirt when a trench they were excavating collapsed in the Waikato backblocks, in what WorkSafe says was a preventable incident.

Safety alert: leg entrapment in hoe rotor

After completing a stabilising run of approximately 180 meters, a machine operator returned a rotary hoe to the park-up area. Once in the park-up area, the operator raised the rotor and hood and, with the rotor and hood elevated, approached the powered turning rotor and kicked at some dirt on the lower skid to clear debris from the skid.

Updated safety alert - TTM and level crossing interface

KiwiRail has updated its safety alert about the interface between temporary traffic management and level crossings, which includes examples of confusing signage approaching level crossings and guidance on temporary traffic management plans.

Safety alert: worker injured after ingesting fuel

Auckland Transport issued a safety alert after a worker ingested an unknown quantity of unleaded fuel while priming a siphon to transfer fuel from one portable generator to another.

The operator of an AirVac system was harmed when his leg was sucked into the live unit.

KiwiRail has issued a safety alert about temporary traffic management interfaces with railway level crossings.

Updates: Regional return to COVID-19 Alert Level 2 and 3

New Zealand is moving to COVID-19 Alert from 12pm on 12 August. The Auckland region will move to Alert Level 3. It will remain at Alert Level 3 for at least 3 days. The Auckland region is everything in the Auckland Council boundary — from Wellsford in the north to Pukekohe in the south. The rest of New Zealand is moving to Alert Level 2.

Safety alert – falling trailer ramps

Inframax has issued an industry-wide safety alert following a fatality with a falling trailer ramp on a wash apron earlier this month. Two versions of the alert have been issued (see below), one for semi trailers and another for full trailers.
